Ray of Hope Nutrition is an in network provider for Aetna and Cigna. If you have one of these two insurances, please follow these steps to determine your coverage.

1. Call your insurance provider (the phone number on your insurance card) and ask the representative what nutrition services are covered

2. Ask how many visits are covered per year

3. Acquire a reference number to provide Ray of Hope Nutrition when scheduling

*If the representative asks for a procedure code, (CPT code) they are

            CPT 97802 (initial nutrition visit)

            CPT 97803 (follow up nutrition visit)

If you are not in network with these two insurances, the out of pocket fee will apply. A superbill will be provided at the end of the appointment. You can submit the superbill to your insurance company for reimbursement. The superbill does not guarantee reimbursement.  

Out of Pocket Rate

Initial (60 minute) appointment- 200$

Follow-up (50 minutes) appointment- 130$

Follow-up (3o minutes) appointment- 80$

I accept cash, checks, HSA cards, and all major credit cards as forms of payment. 

Cancellation Policy

If you are unable to attend a session, please make sure you cancel at least 48 hours beforehand. Failure to do so will result in a $50 cancellation fee.
